Radiant Chiropractic’s website, located at www.radiant-chiropractic.com, together with our online scheduling and other services (collectively, the “Services”), are owned and operated by Radiant Chiropractic, LLC (referred to herein as “Radiant,” “we,” or “us”). Please read these terms of use (“Terms of Use”) carefully because they set forth the important terms you will need to know about the Services. Your use of the Services constitutes your agreement to all such terms, conditions, and notices. In the event that you do not agree with these Terms of Use, do not access or use our Services.

Use of the Services is governed by these Terms of Use, and by using the Services you agree to abide by these Terms of Use as well as all applicable laws and regulations. The Services may contain links to other websites (“Linked Sites”), including our third-party online scheduling platform. The Linked Sites are not under our control, and we are not responsible for the contents of any Linked Sites, including without limitation any link contained in a Linked Site or any changes or updates to a Linked Site. We provide these links to you only as a convenience, and the inclusion of any link does not imply endorsement by us of the site or any association with its operators. Radiant shall not be responsible or liable, directly or indirectly, for any damages or losses caused or alleged to be caused by or in connection with the use of or reliance on any content, products, services, or other materials available on or through any such website. Radiant is not responsible for the privacy practices of any other website.

No Unlawful or Prohibited Use

As a condition of your use of the Services, you warrant that you will not use the Services for any purpose that is unlawful or prohibited by these Terms of Use. You may not use the Services in any manner which could damage, disable, overburden, or impair the Services or interfere with any other party’s use and enjoyment of the Services. You may not obtain or attempt to obtain any materials or information through any means not intentionally made available or provided for through the Services.

Privacy

By using the Services, you consent to our processing of your information consistent with our Notice of Privacy Practices, available at https://www.radiant-chiropractic.com/privacy-policy, which is incorporated herein by reference.

Service Description

Radiant provides in-person chiropractic care (“Clinical Services”) delivered by a licensed doctor of chiropractic. The website and online scheduling are provided to share information about the practice and to allow you to request and manage appointments. The Services do not themselves constitute the practice of chiropractic or the provision of medical advice, and information made available through the website is general in nature and is not a substitute for an in-person evaluation.

The doctor of chiropractic providing Clinical Services holds all required professional licenses issued by the Idaho Board of Chiropractic Physicians. You may report a complaint regarding the Clinical Services by contacting the Idaho Board of Chiropractic Physicians.

Patients and Minors

Radiant provides care to patients of all ages, including infants and children. A patient who is a minor must be accompanied by a parent or legal guardian, and care will be provided only with the consent of a parent or legal guardian who is authorized to consent to the minor’s care. By scheduling or consenting to care on behalf of a minor, you represent that you are the parent or legal guardian and are authorized to do so.

Payments and Cancellations

Payment is due at the time of service unless other arrangements have been made. We may require a valid payment method to be kept on file to secure appointments and to cover amounts owed, including treatment charges and any missed-appointment or late-cancellation fees. If you need to cancel or reschedule, we ask that you provide advance notice in accordance with our cancellation policy; missed appointments or cancellations without sufficient notice may be subject to a fee. These Terms of Use serve as notice of our cancellation and missed-appointment fees.

Disclaimer of Warranties

THE INFORMATION, PRODUCTS, AND SERVICES INCLUDED IN OR AVAILABLE THROUGH THE WEBSITE MAY INCLUDE INACCURACIES OR TYPOGRAPHICAL ERRORS. CHANGES ARE PERIODICALLY ADDED TO THE INFORMATION HEREIN. RADIANT MAY MAKE IMPROVEMENTS AND/OR CHANGES TO THE SERVICES AT ANY TIME.
